Notices of Privacy Practices

Under the Health Insurance Portability and Accountability Act of 1996, known as HIPAA, we are required to inform our patients about:

How We May Use Your PHI

The following information provides an overview of how we may use PHI in providing treatment and services to our patients.

  • Diagnostic processes, such as reviewing the results of imaging tests and bloodwork
  • Filling and refilling medications at a pharmacy of patient choice
  • Internally amongst our team to coordinate care
  • Externally with primary care providers and specialists to coordinate care
  • Bill and collect payment from insurance carriers
  • Bill and collect payment from third parties, such as authorized family members
